Our hospital employs over 600 caring staff, physicians and midwives providing excellent primary, acute and long term care to citizens of Lanark County, West Ottawa and Renfrew County.

Reporting to the Manager of Patient Flow, Surgical Services & Medical Device Repossessing.

The Patient Flow Coordinator/Social Worker is accountable for oversight of patient flow with the goal of seamless transitions through the continuum of care for patients.

This role is currently a Temporary position that could potentially lead to a permanent Full-time position.

In collaboration with the clinical team, the Patient Flow Coordinator/Social Worker is responsible to provide person centered Social Work services to patients and their families. 

  • Assessment
  • Therapeutic intervention and coordination of discharge planning; in collaboration with the clinical team, patients and families. This role is currently a temporary position that could lead to permanent.
  • Facilitating the collection, analysis and use of patient flow information in order 
  • Identify barriers to optimal patient flow 
  • In collaboration with our team to development strategies to build the organization’s capacity to meet the demand for service and improve client flow throughout the organization.

All responsibilities are expected to be carried out in accordance with the mission, vision and values of the hospital.

Qualifications:

  • Master’s degree in Social Work required
  • Member in good standing with the Ontario College of Social Workers & Social Service Workers
  • Advanced computer skills with database and spreadsheet programs (Excel), word processing programs (Word), and presentation programs (PowerPoint)
  • Minimum of 1-3 years of clinical social work experience
  •  

Competencies and Personal Attributes:

  • Demonstrated experience in the assessment and evaluation of the individual needs of clients through the use of social work knowledge.
  • Assist individuals and families in achieving optimum psychosocial and social functioning as they transition from an acute care setting.
  • Analytical skills to: assess processes, identify barriers and develop comprehensive recommendations for improving patient flow; recognizing both internal and external resourcing and demands on services.
  • Able to critically appraise literature to inform practice, including policy development.
