POSITION: Part-Time (Permanent) – Hopkins Community Participation Supports/Direct Support Professional

HOURS:    35 hours per week (Days, Evenings, Weekends)                              

NATURE OF POSITION:

Hopkins Community Participation offers person centred supports to adults with diverse abilities.  The successful candidate will report to the Community Participation Manager and work out of the C.D. Hopkins Centre in Beamsville.  Regular hours will be Monday to Friday. Training to provide support in 24-hour care sites will be provided, and if necessary staff will be temporarily assigned to provide support as necessary.

KEY RESPONSIBILITIES:

Provide physical and emotional support to participants as needed; This may include personal care related to hygiene needs. Make provisions for participant’s safety. Plan community activities and opportunities as it relates to participants interests and goals. Assume the role of Primary Direct Support Professional for 1 to 3 participants.

DUTIES WILL INCLUDE but are not limited to:

  • Provide support for the care and needs of the participant.
  • Design and implement appropriate recreation programs for adults with special needs, using C.D. Hopkins as a home base.
  • Anticipate future requirements of participants (supplies, personal and medical).
  • Maintain a professional attitude with participants, their families and caregivers.
  • Assist participant(s) to set personal goals, develop, schedule and track goal progress.
  • Support the participants in a wide variety of locations and environments throughout the community as it is an integral part of the program.
  • Cooperate with other staff members in establishing a supportive and positive working environment.
  • Advocate on behalf of the participants in an appropriate manner.
  • Dispense medication and maintain medication logs.
  • To participate in annual ISP reviews for participants when requested.
  • Cooperate in maintaining complete and confidential records for each participant.
  • Participate in the orientation and training of new staff, placement students and volunteers, as directed by the Program Supervisor.
  • Attend and participate in staff meetings as required.
  • Use petty cash and participant finances appropriately following all Internal Financial Policies put in place.
  • Follow policy and procedural items established by the agency for the health, safety and welfare of the participants in the program, staff, volunteers and members of the community. Follow all policy and procedural items established by the agency, as they pertain to employee conduct and performance. Follow all Occupational Health and Safety policies

JOB REQUIREMENTS:

  • Performing/Assisting with personal care
  • Frequent bending, twisting and pushing
  • Valid First Aid/Level C CPR & NVCI (Non-Violent Crisis Intervention) 
  • The candidate must have received, at minimum, a high school diploma
  • A valid ‘G’ driver’s license
  • Must be reliable, flexible and able to work independently as well as in a team
